In the winter of 2020, I suddenly developed anal itching. I started dieting and became obsessed with my weight, so much so that I gave myself the strength to have a baby in the bathroom just to lose a few more grams, which was the beginning of a tragedy.

When I visited the hospital, they said I had a third-degree complex hemorrhoid. Since I was suffering from pain that was so severe that my daily life was completely ruined, I decided to have surgery right away without much thought.

I thought that if I had the surgery, I would return to my original Eastern Europe, but after the surgery, I realized that once I went under the knife, I would never be the same again.

I thought I was making slow but steady progress, but one day, I had a stool that was a little thicker than usual and my anus was severely torn. And from then on, I started suffering from severe anal itching again, incomparable to before the hemorrhoid surgery.

At first, only my anus was itchy, but after a few months, I told the doctor that my vulva and buttocks were itchy and scratchy whenever there was a gap, and that it seemed like I was leaking liquid stool a little. The doctor said that the itching in my vulva and buttocks could be due to vaginitis, and that it was not fecal incontinence, but rather the discharge from scratching, and prescribed me oral antihistamines and steroid ointment.

In conclusion, antihistamines and steroid ointments were not effective. Antihistamines and steroids are not treatments. They are only symptomatic treatments that suppress the immune response and relieve itching. Steroids are also not for long-term use.

At that time, I should have been prescribed antifungal or antibiotics because it was not a simple wound itch but an itch caused by a secondary infection. I gave up on outpatient treatment because I thought there was no special way to treat it even if I went to the hospital, but I remember that the itchiness suddenly eased a lot with a 1,000 won disinfectant I bought at the pharmacy.

But that was it. It didn't get any better. Another year passed. It's been 4 years since I had the hemorrhoid surgery, and I'm still suffering, and I can't even remember what it felt like when I was healthy.
Then I recently found out about Bepanthen ointment. Diaper rash, lacerations, acute and chronic dermatitis, eczema, skin ulcers, wounds... This seemed like the ointment for me.

Bepanthen ointment comes in two sizes, 30g and 100g. I bought the 30g size just in case it didn't work. The price varies depending on the pharmacy, but it usually costs around 11,000~14,000 won.

Bepanthen ointment |
Bepanthen ointment's main ingredient is dexpanthenol (or D-panthenol), which is a precursor of vitamin B5 (pantothenic acid). Pantothenic acid is necessary for the synthesis of coenzyme A, which plays an important role in the metabolism and synthesis of various substances in the body. Dexpanthenol is better absorbed into the skin than pantothenic acid and is converted into pantothenic acid in the body, promoting fibroblast proliferation and accelerating re-epithelialization to moisturize the skin and restore the skin barrier to heal wounds. Since it is a non-steroidal ointment, it has few side effects from long-term use.

Dexpanthenol itself has strong hygroscopic properties and is effective in moisturizing the skin, but it also contains excipients such as white wax (bleached beeswax) and lanolin (wax extracted from sheep wool), so the ointment has an overall very thick and sticky formulation.

Actually, I tried using ointment containing zinc oxide and Vaseline, but they don't wash off well with water, so I have to wash my skin with soap every time, which actually irritates the skin and doesn't really relieve the itchiness.

Bepanthen ointment is applied thinly after a sitz bath. It is sticky, but if you gently press with a tissue to remove the excess oil, only a soft feeling remains, so there is no inconvenience in moving around. Since it is washed off to some extent with water, there is less burden of having to apply soap every time.
It has definitely improved since I started using it for a week. I have been lying on my side for 4 years because pus flows into my buttocks and it quickly becomes swollen, but after applying Bepanthen ointment, I was able to lie upright for the first time in just one week. The itchiness has also been significantly reduced, so the number of times I sit in a sitz bath per day has been reduced to less than two. It is said that it can take at least 3 weeks to up to 6 weeks for the wound to heal, depending on the severity of the wound.
